A program for New York residents, designed to engage you in conversations about end-of-life care choices. In highlighting the importance of making choices before circumstances necessitate taking action, CFD will offer practical guidance to anyone towards filling out the two most vital NY State forms—a Health Care Proxy and a Living Will.

These Advance Directives protect your personal healthcare choices in the case of an unexpected event or critical illness that may compromise your ability to speak or verbalize your wishes. Most of us realize the value of having these accessible and essential documents. Yet we put it off, way out into the future, as if there’s no rush to complete them.
